Analisis Faktor-Faktor yang Berhubungan Dengan Pencapaian Dosis KT/V Metode Online Clearance Monitor (OCM) Pada Pasien Hemodialisis

Latar belakang: Hemodialisis adalah terapi pengganti ginjal utama di Indonesia, dengan keberhasilan terapi ditentukan oleh kecukupan dosis dialisis (Kt/V). Teknologi Online Clearance Monitor (OCM) memungkinkan pemantauan Kt/V secara real-time, namun pencapaian dosis optimal masih dipengaruhi oleh faktor klinis dan teknis selama prosedur berlangsung. Tujuan: Penelitian ini bertujuan menganalisis faktor-faktor yang berhubungan dengan pencapaian dosis Kt/V menggunakan metode Online Clearance Monitor (OCM) pada pasien hemodialisis. Metode: Penelitian deskriptif korelasional dengan pendekatan cross-sectional pada 227 pasien hemodialisis reguler di Kota Bekasi yang menggunakan mesin dengan teknologi OCM. Variabel independen: luas permukaan tubuh (V urea), akses vaskular, kecepatan aliran darah (QB), efektivitas laju darah (EBF), dan waktu terapi. Variabel dependen: pencapaian dosis Kt/V berbasis OCM. Analisis data menggunakan regresi ordinal (bivariat dan multivariat). Hasil: Pencapaian dosis Kt/V optimal belum merata. Analisis bivariat menunjukkan hubungan signifikan dengan luas permukaan tubuh, akses vaskular, kecepatan aliran darah (QB), efektivitas laju darah (EBF), dan waktu terapi. Hasil multivariat menunjukkan QB sebagai faktor paling kuat, diikuti waktu terapi, EBF, dan luas permukaan tubuh, sedangkan akses vaskular tidak berpengaruh signifikan setelah dikontrol faktor teknis lainnya. Kesimpulan: Pencapaian dosis Kt/V berbasis OCM dipengaruhi oleh luas permukaan tubuh (V urea), kecepatan aliran darah (QB), efektivitas laju darah (EBF), dan waktu terapi hemodialisis. Optimalisasi ketiga parameter teknis tersebut secara individual sangat penting untuk mencapai kecukupan dialisis. OCM berperan strategis sebagai alat pemantauan clearance difusi yang mendukung praktik keperawatan hemodialisis berbasis bukti.
